代理人的個人資料 / Personal Particulars of the Agent
中文姓名 - The agent’s full name in Chinese characters. Add “先生” (Mr), “小姐” (Miss), “女士” (Ms/Mrs) in brackets. 英文姓名 - Must be printed in BLOCK letters exactly as shown on the agent‘s Hong Kong identity card. Also state Mr/Miss/Ms/Mrs. 香港身分證號碼 - The alphanumeric code of the agent’s Hong Kong identity card. Do not omit the bracket digits. 香港住址 - A residential address in Hong Kong. A post office box is not acceptable. This must be the agent’s usual place of residence, not a business address.

A photocopy of the agent’s Hong Kong identity card must be attached. The form states this explicitly; failure to enclose it will invalidate the notification.

The person signing is the owner or partner of the business, not the agent. Provide: - Name of Signer (print) - Designation - tick one box only: “東主” (Sole proprietor) or “合夥人” (Partner). If the business is a sole proprietorship, the sole proprietor signs; if a partnership, all partners must sign, or one partner may sign but only if all other partners have also signed the same form or a separate continuation sheet is attached for each additional partner. A common error is signing as “Director” - this form is for unincorporated businesses only. - Passport number and place of issue - required only for non-Hong Kong residents acting as signer. This is often left blank by mistake when the signer is a resident; if the signer holds a foreign passport, fill both fields. - Date - the calendar date of signing. Use Day/Month/Year format.
