- Part D - Declaration: A signed statement confirming the information is true and the stamps have not been used or previously claimed for allowance.
The form must be completed in English or Chinese. The applicant must sign the declaration before a witness, who must also sign and provide their name and contact details.

How to fill out Form IRSD25
Page one of the official form. Every field named below appears on it in the same order.
Name, Tel. No., Address
Enter your full name, telephone number, and correspondence address. The name should match the name under which the stamps were originally purchased or the name of the claimant.
Date, Signature with chop
Enter the date you are making the requisition. Sign and affix your company chop (if applicable) in the space provided. The date here is the date of your request, not the date of any prior stamp purchase.
Description of Documents & Reasons, Quantity, Duty (on each), Total
This is the main body of the requisition. For each type of spoiled or unwanted stamp, provide:
- Description of Documents & Reasons: Identify the documents (e.g., “Agreement for Sale and Purchase”) and state the reason the stamps are spoiled or unwanted (e.g., “transaction aborted,” “stamps damaged in handling”). Be specific; the Stamp Office will assess the validity of the reason.
- Quantity: Enter the number of stamps or documents being returned. Do not include the total here.
- Total: Multiply the quantity by the duty per stamp and enter the total value in dollars and cents.
If you have more than a few lines, use a continuation sheet with the same column headings. The total of all lines should equal the value for which the Allowance Ticket will be issued.
Allowance Ticket Section (Bottom Half)
This section is completed by the Stamp Office after they process your returned stamps. Do not fill it in yourself. It will show the total value of the allowance and the machine-printed date of issue.
Stamps in lieu / Refund by cheque (Tick box)
After the Stamp Office issues the Allowance Ticket, you must tick one box to indicate your choice:
- Stamps in lieu: You want replacement stamps of the same total value.
- Refund by cheque: You want a refund cheque for the total value.
Tick only one box. If you tick both, the Stamp Office may reject the request.
Name, Signature with chop (Allowance Ticket section)
After ticking your choice, print your name and sign (with chop if applicable) in the space provided. This signature confirms your election.
Important: Lapse of Allowance
The allowance ticket lapses one year from the machine-printed date shown on the ticket (the date the Stamp Office issues it). If you do not collect replacement stamps or cash the refund cheque within that year, the allowance expires and you lose the value. Do not confuse this with the date you signed the requisition.
Common Mistakes
- Dates: The date on the requisition is your request date. The machine-printed date on the allowance ticket is the issue date. The one-year lapse runs from the machine-printed date, not your request date.
- Mutually exclusive boxes: You must tick either “Stamps in lieu” or “Refund by cheque” - never both.
- Continuation sheets: If you have many items, attach a separate sheet with the same column headings and reference it on the form. Do not cram multiple items into one line.
- Signature: The requisition must be signed. A chop alone is not sufficient; both signature and chop are required where indicated.
