Enter the date the mortgagee ceased to be in possession. Format: DD/MM/YYYY. This is the key date: the form must be delivered to the Registrar within 7 days after this date (see Note 1 of the filing instructions).

If space is insufficient, use the Continuation Sheet (the separate page titled "NM4 Continuation Sheet") and enter the total number of continuation sheets in the box "本通知书所包括的续页数目 Number of Continuation Sheet(s) included in this Notice".

If the mortgagee is an individual, they must sign personally or by their authorized representative. Enter the 8-digit business registration number (BR number) from the company’s Business Registration Certificate issued by the Inland Revenue Department. Do not include the hyphen or the digits after it.
