The Registry will stamp the form and return a certified true copy on request. 3 委任為接管人或經理人的日期 Date of Appointment as Receiver or Manager
填寫該人士最初獲委任為接管人或經理人的日期。此日期應與相關的表格 M5 或 NM5 所申報的委任日期完全一致。格式為日、月、年(DD/MM/YYYY)。
4 文書設立日期/法院命令日期及文書描述/法院案件編號
