Business Registration Number

Enter the business registration number as it appears on the current BR certificate. This is the number assigned to the business by the Business Registration Office.

Business Name

Fill in the exact full name of the business as registered. This must match the name on the existing 3-year certificate.

Effective Year (Notes 1 & 2)

State the year of the certificate you want the revocation to apply to. For example, if your next certificate commences on 1 May 2025, write "2025". The revocation applies to both the main office and all branches. Common error: filers put the current year instead of the year the next certificate starts. Reasons for Late Revocation (Note 3)

Use this box only if you are submitting the notice after the deadline. The deadline is 1 month before the expiry of the current 3-year certificate.
