Check the boxes for the specific forms you need, then write the quantity for each. Note 2 states that you can request a maximum of 5 forms in total per request. The form numbers and their uses are:

- 表格 1(b) - for a body corporate (company) applying for registration.
- 表格 1(c) - for a partnership (other than a limited partnership fund) or other unincorporated body.
