How to fill out Form IRSD106
Page one of the official form. Every field named below appears on it in the same order.
1. Particulars of Exchange Participant
Name in English / Name in Chinese (Notes 2 & 3)
Enter the full official name. The "Old" column should show the name currently registered with the Stamp Office. The "New" column must contain the updated name exactly as it appears on the amended Business Registration Certificate (Note 2) and, if the name has changed, the Certificate of Change of Name (Note 3).
Common error: Failing to attach both certified true copies when the name has changed.
Address (Note 2)
Provide the complete correspondence address. The "Old" column is the address currently on file; the "New" column is the updated address exactly matching the amended Business Registration Certificate.
Contact Person and Tel. No.
Provide the name and direct telephone number of a person the Stamp Office can contact about this notification.
2. Particulars of Staff Authorized to Sign Endorsements on Instruments of Transfer
A. Outgoing Staff
- Name (English / Chinese) and H.K.I.C. No: Full name and Hong Kong Identity Card number of the staff member whose authorization is being cancelled.
- Effective Date: The date from which the cancellation takes effect. This must be on or after the date the form is signed.
B. Incoming Staff
- Name (English / Chinese) and H.K.I.C. No: Full name and HKIC number of the newly authorized staff member.
- Specimen initial appearing on endorsements: The exact mark or signature the staff member will use when endorsing instruments of transfer.
- Effective Date: The date from which the authorization becomes valid.
3. New Specimen Endorsement on Instrument of Transfer
This section is completed only when adding new authorized staff. The "New Specimen" must be pasted or reproduced in the provided area. It should be identical to the specimen initial entered for each incoming staff member in section 2B.
Declaration Block
Authorised Signature and Company Chop
Must be signed by a person authorized by the Exchange Participant to sign on its behalf. The signature must be accompanied by the company chop (official seal).
Name of Signatory / Designation
Print the full name and official job title of the person signing.
Name of Exchange Participant
The exact name of the Exchange Participant as registered with the Stamp Office.
Exchange Participant ID
The identification number assigned to the Exchange Participant by the Stock Exchange or the Stamp Office.
Business Registration No.
The Business Registration number shown on the BR certificate. If the number has changed, attach the amended certificate.
Date
The date the form is signed.
Email Address
A working email for correspondence about this notification.
Note 1 (on form): Cross out (delete) any section that has no changes, do not leave blank sections.
